Founded by the National Minority Quality Forum (NMQF), SHC identifies health patterns in communities around the United States, over time. Disease incidence, prevalence, morbidity, and mortality, mapped together with social, cultural, and environmental determinants of health and indicators of health care access, safety, and costs are extracted from multiple sources, including national surveys, public and private administrative claims, clinical registries, and more.
